If you are a new patient to Swanson Dental Group, we politely request that you arrive 30mins prior to your very first appointment at our Reno office in order to fill out paperwork and allow us time to get your information into our system. If you would like to save time, you may complete the following forms prior to your first appointment:
Insurance
The cost of your treatment will vary depending on your individual dental needs and treatment plan. We will always discuss the cost of your treatment and any available payment options before you begin, so that you can make the best choice for you.
We accept many dental insurance plans and as a courtesy, we will file your insurance claim for you whenever possible. Otherwise, we are happy to assist you in filing your claim or answer any questions you may have about our practice and your dental insurance coverage.
In addition to dental insurance, we accept major credit cards, checks and cash. No-interest dental financing is accepted through CareCredit.
We understand that the world of dental insurance can be very confusing, so please use us as your resource when trying to navigate through the insurance maze. It is also very important that you inform us of any changes to your insurance, as it will directly impact your out-of-pocket expense.
Unless you have made other arrangements in advance, we require payment for services at the time of your visit.
